Appointments and Fees
Consultations are by appointment only. Please phone reception staff on (03) 9005 3888 to make an appointment.
You will need a current referral from your general practitioner to claim the maximum medicare rebate. Please bring this with you to your appointment. Faxed referrals sent in by your GP prior to your appointment are preferable. You will receive an SMS reminder around two days before your appointment, if you have provided your mobile phone number: please reply with Y to confirm. If you need to cancel or change your appointment, please call at least 24 hours in advance. This allows us to fill the appointment time with patients on our waiting list. Fees may apply with failure to notify us due to cancellations.
Please call reception staff if you are running late, so appropriate arrangements can be made to accommodate you with minimal disruption to other patients.
Patient Registration Form
All new patients must complete and sign a Patient Registration Form before the first appointment. You can either:
- Download and Print the Patient Registration Form
- Complete an online version of the Patient Registration Form.
This is a private specialist practice, and consultations will include an out of pocket cost. Dr Aviva Nathan and Dr Deirdre Percy each have their own fee schedules for both obstetric and gynaecology appointments. Dr Nathan and Dr Percy do not bulk bill.
Dr Nathan and Dr Percy both provide a service to public patients at Monash Medical Centre, and patients who wish to be seen by a specialist with no out of pocket cost may be referred to the public hospital clinic.
Private health insurance, including hospital cover, is preferable if you require an operation. Alternatively you can be booked onto a public hospital waiting list with Southern Health.
Payment is required on the day of consultation.
